Our patients experience transformational changes in their life: Better relationships, improved sense of wellbeing, higher self esteem, reduction of stress levels, and greater clarity about their purpose and direction in life.

The first step is identifying those who may benefit from a personliazed mental health care plan, including those exhibiting signs of:

  • • Depression, loneliness, withdrawal
  • • Anxiety
  • • Appetite or sleep disturbance
  • • Hallucinations, delusions
  • • Difficulty adjusting to the nursing home/care community
  • • Previous mental health diagnosis
  • • Psychosis
  • • Agitation, irritability
  • • Confusion, memory loss, dementia
  • • Behavioral problems (Verbal or physical aggression, non-compliance with activities of daily living, diet and medication)
  • • Family related issues
  • • Issues of death, terminal illness
  • • Grief, bereavement
  • • Alcohol and substance abuse
